Lifewalks Privilege Card

Please send me an application form for a Lifewalks Privilege Card.

My GP or consultant has said it is safe for me to join lifewalks.

I have not seen my doctor but I am not aware that I have any medical

condition which would put me at risk if I walk faster than usual.
